Chronic heart failure, ischemic heart disease (reduced blood flow to heart), arrhythmias (irregular heartbeats), congenital heart disease (born with heart defect), heart transplant, aortic aneurysms, vein problems in legs (chronic venous insufficiency), peripheral arterial disease (PAD - poor circulation in limbs).
Medical records over at least 3 months showing your condition, treatment, and response. Evidence that your condition severely limits your ability to function despite treatment. This includes symptoms, exam results, and specific tests (like ECGs, sometimes exercise tests). A heart transplant automatically qualifies for 1 year.
